As part of a pioneering national initiative, Operose Health has teamed up with ImproveWell to give primary care teams a stronger voice in shaping the future of care.
Spanning 82 sites across the UK, including GP practices, Primary Care Hubs, Emergency Department Streaming and Urgent Care Services, the three-year programme aims to gather ideas, share best practice and improve outcomes for over 700,000 patients.

NHS Southend and NHS Castle Point & Rochford CCGs were the first to pilot the ImproveWell platform nationally over a 12-week period.
The goal was to strengthen feedback loops between GP practices and the CCGs - enhancing support and enabling teams to better address workload pressures.
